Use of and training in:
- Wheelchairs, mobility scooters, standing frames, transfer boards, hoists.
- Assistance with home-based physiotherapy support as prescribed by the physiotherapy team and rehabilitation exercises.
- Help accessing accessible transport, taxis or travel cards.
Clinical & Specialist Nursing Care
For individuals with complex physical health needs:
- PEG feeding, catheter care, ventilator support, pressure area care or medication management
- Input from physiotherapists, occupational therapists, and community nurses as part of the Multi-disciplinary team.
Assistive Technology & Environnmental Adaptations
Support to access, install and learn to use:
- Voice-activated devices, smart home systems, powered beds, communication aids.
- Door openers, ramps, adapted kitchens, grab rails, stairlifts.
- Home modifications in collaboration with OTs and housing services.
Independent Living Skills Development
Tailored life skills training:
- Cooking, cleaning, laundry using adapted tools or support.
- Budgeting, managing finances, shopping independently.
- Using online services, mobile phones or accessible apps.
Social Inclusion & Community Access
Support with:
- Attending work, education, volunteering, social events.
- Rebuilding confidence after injury or illness.
- Reducing social isolation through befriending schemes or peer support.
